What this unit teaches

What did not happen: zou hebben

An incident analysis is mostly about things that did not happen: what would have gone wrong, what almost did. The form is heavy but fixed — had plus participle in the if-clause, zou plus hebben or zijn plus participle in the other half. You need it to produce it, not just to follow it, because it is the language of every improvement proposal.

Als ik had gezwegen, was die melding er nooit geweest.

Facts instead of judgements

A report full of self-blame is useless: nobody can fix «I was careless». Describe the step, the time and the circumstance, and let the committee find the pattern. This is also why the passive from unit 25 matters here — it puts the process in the sentence instead of the person.

Telling the patient: four sentences

Say what happened first, plainly, before any explanation — an explanation that comes first reads as a defence. Keep gelukkig and eigenlijk out of it: they minimise, and the patient hears that. Four sentences, in this order, and then stop talking and let them respond.

1. Ik heb u het verkeerde medicijn gegeven.
2. Het gaat om één tablet.
3. Wij houden u vandaag extra in de gaten.
4. Het spijt mij.
